In my gut, I knew it was premature. Gram-negative rods took hold overnight. She is feverish and sick again. Whatever this one is (I’m guessing Proteus), it’s apparently not affected by Tigecycline, so they’ll be changing her to a different antibiotic later today.

Yet another update

Barbara’s blood cultures have turned up the culprit, and the plan now is to narrow in on this with a specific agent, rather than the initial broad spectrum stuff. Prognosis is good. She is expected to recover, though this (like other instances before) will have weakened her again. Her fever is more controlled and she is resting more comfortably.

ETA: She is doing much better, and has been almost fever free for the last couple of hours. For those asking, the culprit is yeast in blood (candida sepsis) and urinary tract. She is being treated with broad spectrum antibiotics through the night, with a powerful IV anti-fungal (micafungin) ordered for the AM.

She actually just woke up shivering from the cooling blanket. It has been discontinued for now.

Short update

We are still waiting confirmation, but it appears she may be fighting sepsis again. Fever over 104, heart rate high, sleeping pretty much all day. Pneumonia got her down here to the ICU, but it’s the sepsis that will keep her.

Good news

lasarina is returning to the LTAC tomorrow. That means she is stable enough that they can’t keep her in this hospital. The surgeon has agreed to move forward. Her new surgery date is 8-Feb-2010.

Found out why I was wrong about the antibiotics

lasarina was diagnosed with strep in her PICC line. So the plan to change the antibiotics changed with the new data. She’s comfortable enough right now. The surgeon has been really put off by this, so energies toward that would be very welcome.

Progress

lasarina is out of the IMU and into a regular room. She has been changed from IV antibiotics to oral ones. Still hoping to get the surgery done during this admission. Don’t know when we will have another date.

ETA: I seem to have misunderstood the part about antibiotics. They just hung more IV ones. (27-Jan 10:15AM CT)

This morning’s update

Yes, I know it isn’t technically morning any more. But if you had been up until almost 3 am, you would be running a bit behind too.

The Terrible, Horrible, No Good, Very Bad Day. OK, slight exaggeration. Still… I woke up at about 6:15 am to a commotion here in lasarina‘s room. They were trying to draw arterial blood, I think. I was pretty drowsy. She had spiked a fever of 101.3, and they needed blood for cultures. Can you guess where this is going?

Yep, more sepsis. PICC line will be removed as soon as they can find a surgeon to place a central line to replace it. Surgery tomorrow? Not with sepsis active. Postponed, again. Her attending says that this time, he is not going to let them send her back to Kindred between now and whatever new surgical date we get. He’s going to get this cleared up quickly, and get her back on the surgical schedule. I hope he can do just that.

Damn it, we are getting really tired of this.

On the road again…

lasarina is currently undergoing the last planned preoperative test. She has already been evaluated by Kindred, and we have been informed that she will return to the LTAC as soon as all the paperwork is in order, which will be likely tomorrow or Saturday. Then it’s going to be “sit and wait” for a surgery date to be scheduled. That will likely be after the New Year starts.

What’s next on the agenda

First, we all need to wish lasarina a very happy birthday today! I am far too much the gentleman to say how old she is, so don’t even ask!

And now that the staph infection is coming under control, and her probable surgery date is therefore after the New Year, we’ve been told that lasarina will probably have to go back to the LTAC for the remainder of this year. It’s an issue of “not sick enough to stay at acute care hospital” under insurance guidelines for that interim. We don’t know exactly when that will happen, but it’s most likely going to be sometime next week. So, really, nothing surprising in that.

She did get some really good news last night though. Her surgeon dropped by …
